SSM Health at Home is proud to be a United Way of Dane County partner agency.

Our organization’s United Way Campaign Committee is tasked with promoting the United Way and soliciting funds from our employees during an annual campaign.

Meals On Wheels

United Way of Dane County provides financial support for SSM Health at Home’s Meals On Wheels program. All Meals On Wheels clients are evaluated for nutritional need, homebound status, and financial hardship. United Way of Dane County provides funding for meals and associated program costs for those clients who meet qualification criteria and who do not qualify for other funding sources.

Clients receiving United Way of Dane County funding for their meals receive a monthly letter detailing the cost of their meals for the month and are asked to contribute if they are able, however, donations are not required to continue to receive funding for those clients who qualify.

SAFE at Home

The SAFE at Home program receives funding from United Way of Dane County to support a falls prevention program for seniors residing in Dane County. Base funding covers administrative costs to run the program so all Dane County seniors in need can access the program. SSM Health at Home also receives additional funding through a Humana grant to fund adaptive equipment or home modifications that might be recommended as part of the assessment.

We are currently in the process of developing a donation program for our SAFE at Home clients. The hope is to reach out to clients who have received an assessment asking for a donation to support the program by providing additional funding for adaptive equipment and home modifications for future clients.
