SSM Health at Home’s palliative care services improve patients’ quality of life by helping relieve the pain and symptoms caused by serious or chronic illnesses. Palliative care can be provided at any stage of a serious or chronic illness, including at the same time as curative and life-prolonging treatments.

Supporting Patients & Caregivers

Your SSM Health at Home care team will work with your physician, who remains responsible for coordinating and managing your overall medical care. Your care team may include nurses, social workers, home health aides, physical therapists, occupational therapists, and speech therapists. The team may assist patients and their caregivers in finding community resources that can provide additional support.

Palliative care nurses will make visits during regular hours to monitor and supervise your condition. Patients and caregivers are also provided a phone number that they can call 24 hours a day if the need arises. Calling will put patients in touch with a nurse who can assist over the phone or arrange a home visit if necessary.
