Home Health United's Community Resource Program provides a free medical social worker consultation to anyone facing these challenges:

- Frequent hospitalization and ER visits
- Trouble with pain control
- Frequent falls
- Need to make their home safer
A medical social worker will contact you and if needed, they will schedule a free home health informational visit. This program is for non-emergencies only.

- General in-home services
