Home Telehealth Monitoring Systems: How can they help your Patients?
Home medical monitoring systems have become more popular in assisting patients with a chronic illness to manage their condition at home. Home medical monitoring, used in conjunction with Home Health United nursing visits for teaching and monitoring, helps reduce exacerbation of the disease process and gives the patient more control of their own care.
The machine can check the patient’s weight, pulse, blood pressure and oxygen level. The readings are sent to a Home Health United Nurse via the telephone lines. The information can also be faxed to the physician’s office if requested.
Goal of therapy: Provide symptom identification and early intervention of chronic diseases in order to reduce visits to the ER, reduce re-hospitalizations and increase patient compliance.
Examples of patient conditions that can benefit from home monitoring:
- Congestive Heart Failure (CHF)
- Chronic Obstructive Pulmonary Disorder
- Myocardial Infarction
- Coronary Artery Bypass Graft (CABG) surgery
- Orthostatic Hypotension
Cost of the machine: The monitoring system is provided as part of our nursing visits under Medicare – there is no charge for the monitor as long as the person qualifies for skilled home care visits under Medicare. Private insurances may also pay for the monitor. The patient and/or the family can also purchase the monitoring system.
