How can we help?
The home health team collaborates with the patient’s physician and other medical professionals to develop an individualized plan of care designed to help patients remain at home as they recover from an illness or injury, heal from surgery, or manage chronic conditions.
The home health care team is led by an RN case manager and may also include home health nurses; physical, occupational and speech therapists; medical social workers; and home health aides.
Because every patient’s situation is different, we offer a number of specialty programs including:
Palliative care is a form of home health care available for patients facing chronic or life-limiting illnesses. Care focuses on relieving symptoms, pain, and stress. Like home health services, the palliative care team works with the patient’s physician and may include home health nurses; physical, occupational or speech therapists; medical social workers; aides; and grief and spiritual counselors.
The goal of palliative care is to ease and prevent suffering, helping patients achieve the best quality of life possible. Palliative care is not dependent on diagnosis or life expectancy and can be provided alongside curative treatments. Learn more about palliative care.
Hospice is a special type of care for those with a limited life expectancy who have chosen to stop curative treatment. Most hospice patients prefer to remain at home, and our hospice team helps make that happen.
Hospice is not just for people in the last days or weeks of life. The sooner a patient is admitted, the sooner the care team can begin effectively treating symptoms and managing pain. Our goal is to help patients achieve the best quality of life possible, supporting both them and their loved ones throughout the process.
The hospice team is led by a hospice RN case manager and may also include a hospice medical director; hospice nurse practitioner; hospice nurses; aides; medical social workers; grief and spiritual counselors; volunteers; and physical, occupational or speech therapists. We provide a variety of equipment, supplies, and services to help our patients live as safely and independently as possible at home. Medical equipment specialists are available to assist in navigating insurance requirements on prescribed equipment.

VoiceCare is a personal emergency response system which establishes a connection to help, should the need arise.
Meals On Wheels delivers nutritious mid-day meals for homebound seniors living in Madison, Middleton, Monona, and Sun Prairie.
SAFE at Home provides in-home safety assessments and medication reviews for Dane County residents age 60 and older.
