Position: Physical Therapist Janesville

Home Health United continues to expand programs and with continued growth we are looking for full-time Physical Therapists to work in our Janesville, WI service area. This is a great opportunity to join our highly experienced clinical teams.

Qualifications include:
1) Four year baccalaureate degree in physical therapy or a certificate in physical therapy from a fully accredited physical therapy program.

2) Valid license to practice physical therapy in the state of WI.

3) Two or more years of experience as a physical therapist.

4) Valid WI Drivers License and access to a reliable vehicle.

Responsibilities include:
1) The ability to administer a physical therapy evaluation. The ability to develop a treatment plan based on the evaluation which will be approved by the physician.

2) Provides services requiring substantial specialized care to treat patients to relieve pain, restore function and maximize function.

3) Observes, records, and reports the patient's reaction to treatment and changes in condition. Revises the plan of treatment as indicated.

4) Informs the case manager of changes in patient's status, including making referrals as needed.

Knowledge and abilities include:
1) Knowledge of the principles and practices of physical therapy.

2) Ability to maintain good working relationships with patients and families.

3) Ability to bend, lift and support patients while working with them.

4) Ability to prepare and maintain complete, accurate clinical records.

5) Current working knowledge of Medicare requirements and documentation.

6) Ability to maintain, alter or terminate physical therapy based on systematic monitoring of a patient's progress.
