Home Health United’s Infusion Pharmacy has been recently accredited by the Community Health Accreditation Program (CHAP).
CHAP accreditation demonstrates that Home Health United meets the industry’s highest nationally recognized standards. Rigorous evaluation by CHAP focuses on structure and function, quality of services and products, human and financial resources, and long-term viability. CHAP’s purpose is to define and advance the highest standards of community-based care.

All HHU infusion patients are served by a state-of-the-art sterile compounding pharmacy by a dedicated team of: Pharmacists and Pharmacy Technicians who work collaboratively with the patient’s physician; Infusion Nurse Specialists who are experts in caring for patients receiving IV therapy; Home Health Registered Nurses who monitor patients’ home infusion therapy and provide patient education; Insurance Specialists who handle all financial aspects of patients’ treatment; and Delivery Technicians. The pharmacy team provides individualized patient assessment and progress monitoring throughout the course of treatment to achieve optimal outcomes.
The HHU Infusion Pharmacy is directed by Dr. John Mattern, a PharmD., with 25 years of experience. Dr. Mattern also oversees the SSM Home Care & Hospice infusion pharmacy in St. Louis, Missouri.
In addition to the Infusion Pharmacy, Home Health United provides a variety of services to patients and families across southern Wisconsin, including home health, hospice, home medical equipment, community health and wellness programs, Meals On Wheels, and immunization clinics.
CHAP is an independent, not-for-profit, accrediting body for community-based health care organizations. Through "deeming authority" granted by the Centers for Medicare and Medicaid Services (CMS), CHAP has the regulatory authority to survey agencies providing home health, hospice and home medical equipment services, to determine if they meet the Medicare Conditions of Participation and CMS Quality Standards.
