Medical Social Services

Home Health United social workers play a vital role in the patient-care team.  After a patient is admitted by a nurse, a social worker may come to your home, with your permission, and conduct a thorough psychosocial assessment to help determine your needs and identify any limitations, barriers, or safety issues that might impede your recovery.

A large part of the social worker's job is education of patients and family.  They also help solve problems and may refer you to a social service agency or to other community resources and funding sources.

Their services may include:

  • Home safety
  • Long range planning
  • Education regarding alternative living arrangements
  • Case Management
  • Advanced Directive
  • Community resource planning and coordination
  • Short-term therapy
  • Crisis intervention
  • Financial assessment
  • Counseling with life-style changes
  • Teaching regarding coping with loss and change
  •  Grief counseling
