Trapeze Documentation Needs
The following qualification criteria need to be documented in the patients’ medical record notes to qualify for the prescribed equipment. Note: Justification notes are valid for six months.

All of the following need to be sufficiently documented to support the need for the prescribed equipment.

  1. Explain if the patient needs this device to sit up because of a respiratory condition.
  2. Explain if the patient needs this device to change body position for other medical reasons.
  3. Explain if the patient needs this device to get in or out of bed and why.

Trapeze bar attached to bed with grab bar (HCPC-E0910)
Trapeze bar free standing, with grab bar (HCPC-E0940)
Trapeze bar, heavy duty, free standing with grab bar, (HCPC-E0912) Note: Patient must weigh 250lbs. or more.

Patient notes/justification – Must be documented using your normal dictation process. Letter of medical necessity, notes wrote on letter head or prescriptions are not valid.

Prescription/order requirements – Must contain Patient’s name, date of the order and Start date, detailed equipment description (See above), signature of the ordering physician and a signature date.
