Advance Directives

You have the right to direct your future health needs and determine who will make your medical decisions when you canít. Your wishes can be established and set forth in the following forms:

  1. Living well
  2. Power of attorney for health care
  3. Legal document drafted by your attorney

An advance directive is a written record of your decisions about treatments you want or don't want or about how such decisions should be made if you become incapacitated.

By establishing an advance directive you express your personal wishes, beliefs, and values. Things to consider when making this document: dying, living as long as possible, being kept alive on machines, being independent, and quality of life. While these may be difficult issues to discuss, the conversation is necessary if you want others to follow your wishes.
